Step 1
Please submit your application using our advanced Electronic Health Records (EHR) system.
At the end of the electronic intake form, you will be asked to review our privacy policies and provide consent for treatment.
Step 2
Once we've reviewed your application, we will contact you with a follow-up email within 48 hours to schedule your initial appointment with Dr. Michael.
Step 3
If you have medical records to transfer from another practice, just fill out a release form with them.
After completion, your records can be faxed over to your new Electronic Health Record (EHR) account.
Your First Doctor's Appointment: What to expect!

- After your first appointment, Dr. Michael will work with you to create a personalized treatment plan tailored to your individual needs.

- Follow-up appointments will be scheduled to monitor your progress and make any necessary adjustments to your treatment plan.
- Depending on your situation, anticipate follow-up visits 6, 8, or 12 weeks after your first follow-up visit.

Rupa Health may guide you, answer lab prep questions, and monitor your results. Once received, they'll forward them to us for our review together.
